IOH abides by the Privacy Legislation and the National Privacy Principles, in the way we handle personal information. Accordingly IOH will obtain consent for the collection of health information and for its appropriate use and disclosure. IOH takes all reasonable steps to protect individual and company health information from misuse, loss or unauthorised access, modification or disclosure.
All IOH staff and contractors are aware of the legislation and it’s implications, and all these individuals follow strict protocols to maintain the safety and security of personal information.

IOH may request you to complete an eForm to streamline data collection. Data collected is to inform the service we are providing, communicate with you, facilitate payment of services, meet regulatory requirements or help uniquely identify you and those authorised by you to access the records for that episode of care. Our eForm services reside on servers hosted in Australia with data encrypted. We take best care to ensure data is only accessed by authorised persons. The data is securely imported to our internal health and case management systems and then deleted from the eForm servers.
